Osteochondritis dissecans (OCD) occurs when a small piece of bone and cartilage in a part of a joint separates from the bone around it. The type of osteochondritis dissecans found in kids and teens is known as juvenile osteochondritis dissecans (JOCD). This condition is different than the OCD found in adult populations and requires different treatment plans. OCD is most common in the knee joint, but it can happen in other joints such as the elbow and ankle. The condition can be mild, moderate or severe.
What Is Osteochondritis Dissecans?
Types of Osteochondritis Dissecans (OCD)
It is not entirely known why children develop Osteochondritis dissecans (OCD), but could be loss of blood supply to a piece of bone or overuse of a joint. There are three different types of OCD:
- Mild OCD: A piece of bone has begun to separate from the joint, but this piece is still firmly held in place by a covering of cartilage.
- Moderate OCD: The piece of bone separates more. The covering of cartilage may tear.
- Severe OCD: The piece of bone and covering of cartilage become loose and "float around" in the joint.
Signs and symptoms of Osteochondritis Dissecans (OCD)
- Soreness
- Swelling
- Stiffness
- Feeling that the joint is “locking up”
- Limping
Treatments We Offer
The goal of treatment for Osteochondritis dissecans (OCD) is to heal the separate piece of bone and cartilage back onto the joint. This healing takes time, often up to six months. Treatment can include:
- Rest and stop activity that causes pain. Avoid running and jumping.
- Ice the joint
- Take over-the-counter anti-inflammatory medication including ibuprofen and naproxen.
- Use crutches to lessen stress on the knee or ankle joint, or wear a brace or cast
- The doctor may recommend surgery in moderate to severe cases
